I mean, when you are commencing descent 60 or 100 miles out, it just needs a very small heading change to reach an IAF a few miles away from the old one, and the difference in fuel burn would be negligible - and by the same token, on departure (in jets, at least) you want to get as high as you can as quick as you can and head straight for the IAF at your destination, so with so much open ocean so close at hand, it makes sense to do the climb-on-course out there, without trundling over the city at 250kt to keep the noise down.MikeW
